Please answer all questions to the best of your ability. You will receive a confirmation message on this page, and in an email, when you've completed registration.

Your class spot is not guaranteed until the following criteria are met:

  • complete this form, which includes: 

    • confirming your agreement with Olean Kennel Club's terms and waiver

    • uploading proof of vaccination records from your vet,

  • Submit payment in full for your class (via PayPal, cash or check). All sales final. Class fees are non-refundable. No exceptions.
